Build custom forms for your field team - for visits, inspections, surveys, and more - and see every response in one place.
Forms lets administrators and authorized managers create custom forms to collect information from field employees. Use them for field visits, inspections, surveys, client interactions, or any other field activity where you need consistent, structured information. Field employees fill in the forms they're assigned, and every response, including photos, files, and location, comes back to one central view.
How to Create a Form
Log in to the Admin Panel.
Go to Field Tracking → Forms.
Click Create Form.
Enter the form details.
Add the fields you need (see below), and set each one as Required or Optional.
Save the form.
Field Types
Each form can include any combination of these field types:
Text - written answers
Number - numeric values, such as quantities or readings
Date & Time - a specific date and/or time
Single/Multiple Select - choose one or more options from a list you define
Yes/No - a simple yes or no answer
Image/File Upload - photos or documents, such as site photos or signed forms
Location - the employee's location when filling in the form
Rating - a score, such as for customer satisfaction or site condition
Each field can be set as Required or Optional, with extra settings depending on the field type (for example, the list of options for a Select field).
Form Status
Draft - the form is still being prepared and can't be filled in yet
Active - the form is published and available to assigned field employees

Delete - remove the form permanently
Assigning Forms
Field employees can only see and fill in forms that are assigned to them.

Log in to the Admin Panel.
Go to Field Tracking → Form Response.
From here, you can:
View all submitted responses
Search responses, and filter them by form, employee, or date
